The Health Information Management Department is responsible for the management of the Psychiatric Institute of Washington medical records.

Health Information Management representatives may be reached by dialing (202) 885-5694 between the hours of 8:00 a.m. and 4:30 p.m., Monday through Friday.

Release of patient information requires a signed authorization by the patient that meets the privacy requirements of the District of Columbia and federal regulations. Copies of medical records are available upon written request with a valid patient authorization.
